Análise de fatores associados à saúde mental em gestantes e puérperas no Brasil: Uma revisão da literatura

RESUMO Introdução O surgimento ou agravamento de problemas e transtornos mentais durante a gravidez e o puerpério tem sido amplamente documentado na literatura e contribui significativamente para o aumento da morbimortalidade materno-fetal. O objetivo deste estudo foi realizar uma revisão de escopo da literatura para levantar as principais evidências de estudos brasileiros no âmbito da investigação de problemas de saúde mental em gestantes e puérperas, identificando os principais fatores de risco apontados na literatura. Métodos Realizou-se uma busca sistemática de estudos nas plataformas PubMed e Scielo a partir de estratégia de busca elaborada com descritores e termos-chave. Os processos de seleção de artigos e de extração de dados foram realizados por duplas independentes de pesquisadores. Resultados Ao todo, foram incluídos e revisados 94 artigos. A maioria dos estudos apresentava delineamento transversal e foi conduzida na região Sudeste do país. Depressão (n=51) e ansiedade (n=29) foram os transtornos mais estudados. A mediana da prevalência de depressão pré-natal e pós-natal nos estudos foi de 16,9% e 20,8%, respectivamente, e alguns estudos indicam sintomas de ansiedade em 80% das participantes. A presença de risco de suicídio variou entre 6,3% e 23,53%. Apesar de pouco avaliados nos estudos, o transtorno bipolar e os transtornos psicóticos demonstram ser fatores de risco para suicídio e estão associados a piores desfechos materno-infantis. Conclusões Estes resultados destacam a importância da avaliação da saúde mental durante o pré-natal e a qualificação das políticas de acesso aos serviços de saúde mental na gestação e no puerpério.

Uso de drogas ilícitas entre estudantes de graduação no Sul do Brasil: aumento na prevalência entre 2015 e 2019 e o papel da migração acadêmica

RESUMO Objetivo Avaliar a prevalência do uso de drogas ilícitas nos anos de 2015 e 2019 entre estudantes de graduação de uma universidade pública do sul do Brasil, observando diferenças na proporção do uso dessas substâncias nesse período, bem como os fatores associados a esse comportamento. Métodos Estudo de painel, realizado a partir de duas pesquisas com corte transversal, nos anos de 2015 e 2019. Os dados foram coletados através de questionário autoadministrado. Realizou-se análise multivariada, por meio da regressão de Poisson. Resultados Participaram 1.423 estudantes em 2015 e 996 estudantes em 2019. A prevalência do uso de drogas ilícitas na vida, no ano e no mês aumentou entre os dois anos de estudo e as drogas mais utilizadas foram maconha, seguida por drogas sintéticas. As variáveis independentes associadas ao uso de drogas ilícitas no último mês nas duas pesquisas foram migração acadêmica, ser do sexo masculino, ter entre 18 e 24 anos, morar com amigos, ter um familiar que usa ou já usou drogas, ter um amigo que usa ou já usou drogas, ter usado álcool no último mês e ter usado tabaco no último mês. A prática religiosa foi fator de proteção. Conclusão A tendência do uso de drogas ilícitas mostrou-se crescente. Estudantes que migram de suas cidades para estudar na universidade apresentam maior prevalência de uso de drogas ilícitas no último mês. Diante destes resultados, sugerimos a criação de programas de acolhimento e acompanhamento dos estudantes com especial atenção para aqueles que migram de outras cidades para estudar.

Minimum required information for Psychiatric History Taking: a scoping textbook review

ABSTRACT Objective In psychiatry, History Taking(HT) and Mental Status Examination are the only information source for diagnosis, resulting in reliability and validity issues. Standard Diagnostic Interviews(SDI) were prescribed as a solution for diagnostic unreliability but not used in clinical practice. Our objective is to describe and consolidate the “must be collected data” in psychiatric HT, and reasons why some information must be collected for an adequate diagnostic hypothesis generation. Methods Researchers and lecturers from different countries were consulted about Textbooks of Clinical Psychiatry, as a privileged source of consolidated HT information in psychiatric training, using a snowball methodology, complemented by references review and most sold textbooks. Data were systematically extracted by two authors, and then comprehensively synthesized. Results Twenty-five textbooks, from sixteen different countries, were accessed. We did not find a standard model, nor clear reasons for why HT components were chosen, but the manual’s information grossly overlaps, suggesting a “validated by tradition” model in psychiatry. Conclusion The “must be collected data” are both risk factors and prototype building blocks for mental disorders, with different anti-bias strategies. HT content suggested by textbooks is different from what is addressed by SDI, and a consensual “minimum information” for diagnostic reasoning was described.

Accuracy of the revised Addenbrooke Cognitive Examination (ACE-R) in older adults with low education and mild cognitive impairment: results of a cross-sectional study in two metropolitan areas of Northeast Brazil

ABSTRACT Objective To determine the diagnostic accuracy of the Addenbrooke’s Cognitive Examination (ACE-R) for older adults with low education, without dementia, in two capitals in northeastern Brazil, compared to subjects with MCI. Methods 100 participants were collected from a previous neurological and psychiatric evaluation and were subsequently subjected to the ACE-R. Among them, 18 subjects with amnestic mild cognitive impairment (aMCI), 22 with non-amnestic mild cognitive impairment (naMCI), and 60 healthy controls. Results Optimal ACE-R accuracy yielded excellent values for the comparison between controls and naMCI [Area Under the Curve (AUC) = 0.919)] and controls and aMCI (AUC= 0.921); conversely, very fair accuracy was reported for the comparison between aMCI and naMCI (AUC= 0.578). Conclusions These findings support establishing reliable cutoff scores for cognitive assessment of elderlies with low schooling and cognitive decline, not dementia, taking into consideration ecological and regional variables.

O efeito da Meditação em pacientes incuráveis: Bem-estar, Qualidade de Vida e Controlo Sintomático – uma revisão sistemática

RESUMO Objetivos Através de uma Revisão Sistemática, pretendeu-se avaliar o impacto da Meditação no bem-estar, na qualidade de vida e no controlo sintomático em indivíduos com doenças incuráveis. Métodos Foram pesquisados na Pubmed, Web of Science e Scopus, estudos publicados entre janeiro 2012 e dezembro 2022, em língua inglesa, com Participantes- adultos com doença incurável ou terminal; Intervenções- qualquer tipo de meditação; Comparadores- qualquer tipo de controlo; Outcomes- bem-estar, qualidade de vida e o controlo sintomático; Desenho- ensaios clínicos e estudos controlados randomizados. Seguiu-se a Declaração PRISMA 2020. Usou-se a ferramenta Cochrane Rob-2 para o risco de viés. Fez-se uma síntese narrativa dos resultados. Resultados Oito estudos foram incluídos, provenientes de três continentes: Ásia (n=4), Europa (n=2) e América (n=2). Participaram 682 pacientes; a maioria com câncer (seis estudos). Houve grande heterogeneidade dos estudos, sendo o risco global de viés elevado. Conclusões A maioria dos estudos sobre os efeitos da meditação em pacientes incuráveis mostrou: 1) mais relevância no bem-estar e sofrimento; 2) menos relevância na qualidade de vida, ansiedade e depressão; 3) inconsistência na redução do estresse. É fundamental investir em estudos com amostras maiores, mais rigorosos e com seguimentos mais longos que afiram, com maior robustez, o contributo da Meditação nos resultados em saúde junto de populações vulneráveis.

Effects of mind-body exercises in a patient with mood disorders: a case report

ABSTRACT Introduction The rationale of this case study was to evaluate a subject with anxiety and depression under treatment for anxiety and depression with significant bodily symptoms and physical illness, configuring also a psychosomatic condition. We investigated a new protocol of mind-body exercise (MBE) and its effects on the body-mind relationship through qualitative and quantitative analysis Case Report M.S., 47 years old, diagnosed with depression, anxiety, and several other illnesses and physical complaints, was submitted a weekly seven sessions of MBE. After that, anxiety, depression, and interoception were evaluated. Moreover, affect and arousal scales were administered after each session. Discussion Improvements were identified in all dimensions of interoception, with more pronounced results in not-distracting (pre: 4/ post: 0,25), emotional awareness (pre: 1,6/ post: 5), self-regulation (pre: 2,29/ post: 4,29) and trusting (pre: 0/ post: 4), in addition to reducing symptoms of depression (pre: 17/ post: 14) and anxiety (pre: 29/ post: 24) and increase for positive affect (pre:2,2 ± 3,0/post: 3,6 ± 1,7) e arousal (pre:4,5 ± 1,9/post: 5,9 ± 0,4). Conclusion We conclude that MBE improved interoceptive ability and reduced symptoms of anxiety and depression. Through these perceived and reported changes, the patient was able to learn to deal with stress and anxiety and self-regulate.

Trauma infantil e Transtorno de Personalidade Borderline : uma revisão integrativa da literatura

RESUMO Objetivo Analisar o conteúdo da produção de literatura no que diz respeito a relação entre trauma infantil e o diagnóstico de TPB na vida adulta, a fim de compreender as especificidades das bases etiológicas do TPB. Material e Métodos Trata-se de uma revisão integrativa onde realizou-se uma busca nas bases de dados PubMed, SciELO e Lilacs, utilizando os descritores “TRAUMA INFANTIL E TRANSTORNO DA PERSONALIDADE BORDERLINE” e “CHILDHOOD TRAUMA AND BORDERLINE PERSONALITY DISORDER”. Foram incluídos os artigos completos publicados de 2011 a 2021 em jornais científicos, em língua portuguesa, espanhola e inglesa que retratavam a relação entre trauma infantil e o diagnóstico do TPB. Foram excluídos os artigos sem descrição metodológica explícita de definição da amostra TPB através de escalas validadas e/ou a partir dos critérios do DSM e os estudos de revisão. Resultados Foram identificados 423 artigos, e, após uma triagem, restaram 23 artigos elegíveis. 65,2% das obras incluídas foram estudos transversais e 34,8% estudos caso-controle. Todos os trabalhos abordaram abuso sexual, e, a maioria deles também abordou abuso físico (95,6%), abuso emocional (86,9%), negligência emocional (82,6%) e negligência física (78,2%). Todas as obras evidenciaram associação entre vivências traumáticas na infância e o diagnóstico de TPB. Conclusões O trauma infantil pode ser encarado como fator de risco para o diagnóstico de TPB, assim como associa-se com maior gravidade dos sintomas e pior prognóstico.

Padrões do suicídio na região mais populosa de Alagoas, Brasil, 2016-2018

RESUMO Objetivo: Descrever os padrões do suicídio, relacionando-os com aspectos sociodemográficos, espaço-temporais e relacionados ao suicídio na região mais populosa do estado de Alagoas (AL), Brasil. Métodos: Foi delineado um estudo observacional e retrospectivo a partir de dados de suicídio do Instituto Médico Legal localizado em Maceió (AL), compreendendo os anos entre 2016 e 2018. Por meio de regressão logística binária e múltipla foi determinado um modelo estatístico para identificar fatores associados ao suicídio dentre homens e mulheres. Resultados: Do total de suicídios levantados (n = 172), 71,5% eram de vítimas do sexo masculino. A média de idade nos homens foi de 39,3 anos e de 34,1 nas mulheres. O modelo final de regressão logística identificou que homens pretos/pardos tem 12 vezes mais chances (p < 0,001) de cometer suicídio do que os brancos. Além disso homens tem menos chances de se suicidar por intoxicação exógena (p < 0,001) ou queda intencional (p =0,01) do que por enforcamento. Homens em situação empregatícia apresentaram maior chance de suicídio (p = 0,02). Conclusões: Apesar da complexidade do fenômeno em estudo, foi possível verificar alguns padrões para a região mais populosa do estado de Alagoas. O padrão do suicídio no estado seguiu o cenário nacional, afetando homens, adultos, jovens de etnia negra (preta/parda) em situação empregatícia, além do enforcamento como mecanismo padrão entre os homens. O presente estudo poderá ser base para planejamento de intervenções com foco na prevenção do fenômeno, com foco nos determinantes para o suicídio.

Comportamento de risco para transtornos alimentares, percepção da imagem corporal e consumo alimentar em adolescentes praticantes de judô

RESUMO Objetivo Avaliar o consumo alimentar, comportamento de risco para transtornos alimentares e percepção da imagem corporal em adolescentes praticantes de judô. Métodos Estudo transversal com adolescentes judocas participantes da Federação Paranaense de Judô. Os dados foram coletados por meio de um questionário on-line, contendo os instrumentos Body Shape Questionnaire (BSQ) para avaliar percepção da imagem corporal, o Eating Attitudes Test (EAT-26) para identificar comportamentos de risco para transtornos alimentares e o Recordatório alimentar de 24 horas para análise da ingestão alimentar. A insatisfação com o peso corporal foi obtida pelo relato dos que gostariam de ganhar ou perder peso. Resultados Participaram 57 judocas de ambos os sexos, maioria sexo masculino (54,3%; n=31). Ausência de comportamento de risco para transtorno alimentar (63,1%; n=36) e insatisfação com a imagem corporal (75,4%; n=43) foram prevalentes entre os judocas. Entre os que apresentaram insatisfação com a imagem corporal (36,8%; n=14), observou-se que 9 (15,78%) também mostraram comportamento alimentar de risco para transtornos alimentares pelo teste EAT-26 (p=0,002). Houve associação entre insatisfação com o peso e comportamento de risco para TA (p=0,034), representado pelo EAT positivo para 26,31% (n=15) dos adolescentes. Identificou-se valores de ingestão alimentar insuficientes de energia e carboidratos, bem como consumo elevado de proteínas, e inadequação na ingestão para a maioria dos micronutrientes. Conclusão A maioria dos atletas não apresentou comportamento de risco para TA e estavam satisfeitos com sua imagem corporal. Diate da inadequação no consumo alimentar, destaca-se a importância das ações de educação alimentar e nutricional voltadas para este público.

Conclusão Esta será a primeira scoping review que procura investigar a relação entre a microbiota intestinal e o Transtorno Obsessivo-Compulsivo e Transtornos relacionados. A publicação prévia deste protocolo de revisão irá colaborar para um melhor planejamento do estudo e divulgação da investigação junto da comunidade científica.

Impacto da pandemia de COVID-19 na prevalência de transtornos mentais comuns entre estudantes de Medicina

RESUMO Objetivo: Analisar se houve impacto da pandemia de COVID-19 na prevalência de transtornos mentais comuns (TMCs) entre estudantes de Medicina, investigando os possíveis fatores de risco associados. Métodos: Estudo observacional, desenvolvido com 289 estudantes de Medicina de Salvador, Bahia, realizado antes e durante a pandemia da COVID-19. Utilizou-se um questionário estruturado contendo dados sociodemográficos, acadêmicos e sobre hábitos de vida. O rastreamento de TMC foi realizado por meio do Self-Report Questionnaire (SRQ-20). A coleta de dados ocorreu em duas etapas: a primeira, presencial (período anterior à pandemia) e a segunda, virtual (período pandêmico). Resultados: Não houve diferença na prevalência de TMC entre os momentos antes e durante a pandemia (48,0% vs. 44,5%, p = 0,577). Durante a pandemia, verificou-se aumento do envolvimento religioso (80,6% vs. 92,7%, p = 0,002) e do consumo de bebidas alcoólicas (45,9% vs. 69,1%, p < 0,001). Entre os estudantes, observou-se aumento de TMC entre homens (17,0% vs. 34,1%, p = 0,036); redução entre mulheres (83,0% vs. 65,9%, p = 0,036); aumento entre aqueles com envolvimento religioso (80,9% vs. 92,9%, p = 0,036); e aumento em todos os ciclos acadêmicos (p = 0,039). Conclusão: Apesar de a prevalência de TMC em estudantes de Medicina manter-se expressiva, não foi demonstrado impacto direto da pandemia de COVID-19 em suas taxas. Ademais, a prevalência de TMC foi maior entre os estudantes com engajamento religioso e consumidores de bebida alcoólica.

Carbamazepine in the treatment of bipolar disorder: a systematic review

ABSTRACT Objective: Expand knowledge on the role and efficacy of carbamazepine (CBZ) in bipolar disorder (BD), based on original studies. Methods: The authors performed a systematic review of the scientific literature on the efficacy of CBZ in BD, using the PubMed/MEDLINE, Web of Science (ISI), and SciELO databases. The search terms were: (“carbamazepine”) AND (“bipolar” OR “mania” OR “manic”). There was no restriction on the year of publication. Results: A total of 27 articles were selected. Among the selected articles, 14 evaluated the use of CBZ in the manic phase of BD, two in the depressive phase of BD, and 11 in the maintenance phase of BD. In the studies on the manic phase of BD, CBZ proved superior to placebo (PLA). As for the depressive phase of BD, there were two studies, both with small samples. In the maintenance phase, CBZ was inferior to lithium, and no studies compared CBZ to PLA. Conclusion: The results of the clinical studies suggest that CBZ is effective for the treatment of the manic phase. Regarding the treatment of acute depression and maintenance of BD, the results of the studies indicate that there is not enough data demonstrating the effectiveness of CBZ.
