GLP-1-Rezeptor-Agonisten (GLP-1-RA), bekannt als sogenannte „Abnehmspritzen“, helfen schätzungsweise bis zu 850 000 Menschen mit Diabetes Typ 2 in Deutschland, ihr Gewicht zu reduzieren und den Blutzucker besser zu kontrollieren. Doch trotz ihres Rufes als Wundermittel ist eine individuelle Diabetesberatung nach wie vor unverzichtbar, betont der Verband der Diabetes-Beratungs- und Schulungsberufe in Deutschland e. V. (VDBD). Nur allein mit Medikamenten lassen sich ein Diabetes Typ 2 und mögliche Folgeerkrankungen nicht ausreichend und nachhaltig behandeln. Die Deutsche Diabetes Gesellschaft (DDG) macht in einer aktuellen Stellungnahme auf mögliche Nebenwirkungen und Risiken der Behandlung mit GLP-1-Analoga wie Semaglutid und verwandte Substanzen aufmerksam: Die Wirkstoffe, die nicht nur zur Behandlung von Typ-2-Diabetes, sondern auch zur Gewichtsreduktion bei Adipositas eingesetzt werden, können zu einer Erhöhung der Fruchtbarkeit und damit zu ungeplanten Schwangerschaften führen. Gleichzeitig eröffneten sie Frauen mit polyzystischem Ovarialsyndrom (PCOS) und Kinderwunsch bessere Chancen, Mutter zu werden. Da sie jedoch auch Risiken für die Schwangerschaft bergen können, sollten Frauen sich hinsichtlich einer derartigen Therapie unbedingt ärztlich beraten lassen. Behandelnde fordert die DDG auf, ihre Patientinnen über die bekannten Nebenwirkungen umfassend aufzuklären.

Schwarz1,2 für die Kommission Digitalisierung der Deutschen Diabetes Gesellschaft (DDG)Hintergrund: Das wachsende Angebot digitaler Gesundheitsanwendungen (DiGAs) eröffnet neue Möglichkeiten in der Patientenversorgung, bringt aber auch neue Herausforderungen mit sich. Das DiGA-Verzeichnis des BfArM bietet zwar eine Übersicht über verfügbare DiGAs, doch fehlen bisher leicht zugängliche Hilfsmittel zur Bewertung ihrer Effektivität im klinischen Alltag.Methode: Die Kommission Digitalisierung der Deutschen Diabetes Gesellschaft (DDG) hat einen Kriterienkatalog für digitale Diabetes-Therapien entwickelt, um die Auswahl geeigneter Anwendungen für Behandelnde zu erleichtern und langfristig die Qualität digitaler Therapien zu verbessern. Hierzu wurden im Rahmen von Expertenrunden bestehende Qualitätsstandard und validierte Projekte zur Bewertung unterschiedlicher Aspekte von DiGAs extrahiert und zusammengefasst. Anschließend wurde die Relevanz durch eine Befragung von DDG-Mitgliedern bewertet.Ergebnisse: Insgesamt konnten 83 Items identifiziert und 9 Kategorien zugeordnet werden sowie deren Relevanz bewertet werden. In Diskussionen und der Befragung haben sich vor allem die Kategorien Evidenz und Ziel als besonders relevant herausgestellt.Schlussfolgerung: Dieser Artikel stellt den Entwicklungsprozess sowie die Ergebnisse des Bewertungskatalogs für DiGAs vor. Langfristig soll dieser in der Praxis eine leicht zugängliche Hilfestellung im zunehmend digitalen Versorgungsalltag des Diabetes in Deutschland darstellen.

Lebendimpfstoffe erzeugen eine starke, lang-anhaltende Immunität und haben Millionen Menschenleben weltweit gerettet. Sehr selten jedoch können sich angeborene Störungen des Immunsystems nach einer Lebendimpfung manifestieren. Im vorliegenden Beitrag sollen anhand monogenetischer Immundefekte entscheidende Mechanismen der antiviralen Immunität diskutiert werden. Das Verständnis der jeweiligen pathophysiologischen Grundlagen erlaubt Rückschlüsse beispielsweise auf die Impfstoffentwicklung.

ZUSAMMENFASSUNG
Die STIKO ordnet Menschen mit Diabetes und ggf. chronischen Nierenkrankheiten unabhängig von ihrem Diabetestyp als Risikogruppe ein. Für sie gibt es grundsätzlich keine Impfkontraindikationen, Ausnahmen sind lediglich Patienten nach Organtransplantation oder unter intensiver immunsuppressiver Therapie. Daher sollten empfohlene oder notwendige Impfungen auch bei fehlender Impfdokumentation durchgeführt werden. Bei Impfungen wird zwischen Standard-, Auffrisch- und Indikationsimpfungen unterschieden. Folgende Übersicht basiert auf den STIKO-Empfehlungen zu Indikations- und Auffrischimpfungen bei Erwachsenen mit Diabetes, um einen lebenslangen umfassenden Impfschutz zu erzielen.
